This study aims at a) performing a five-year follow up of ca 1 900 clients and patients in different parts of the treatment system for substance abusers in Stockholm county (health care and social service) and b) comparing the treatment systems in Stockholm county and Contra Costa county, California.The study will analyse the role of treatment for substance use and related problems within different groups in a short (one year) and longer (five years) perspective. It will partly build on already collected data from Stockholm county between 2000-2002: interviews with ca 1 900 persons seeking treatment in different part of the system: their routes to treatment, alcohol and drug consumtpion and related problems, social situation and treatment experiences. The clients have been followed up one year later and this second five year follow up will be the next step. The study will also use a survey in the general population on patterns of substance use, attitudes to alcohol and drugs and treatment experiences, with a second interview among high consumers after one year. This data will be compared with a similar data set from Contra Costa, with initial interviews, one and five year follow ups and a general population survey. Some of the main research questions are: Are the Swedish clients/patients more marginalised than those in Contra Costa? What is the effect of societal pressure to seek treatment for different groups? What is the role of number of treatment contacts for the development of substance use in short and longer term? Do women get different treatment than men and is the role of treatment in relation to other social support different for men than for women? What is the role of treatment within different ethnic groups in the two societies? What is the importance of available psychiatric care? The study can give a useful perspective on the role of the Swedish alcohol and drug treatment system in the rehabilitation of different groups of substance users.
